
If you’ve been told the only way to fix Achilles tendonitis is to stop running completely… you might actually be making your injury worse.
One of the biggest myths in the running world is that you can’t run with Achilles pain.
In reality, for most runners, total rest doesn’t build a stronger tendon — it reduces its capacity. And that’s why so many runners get stuck in the frustrating cycle:
Pain → Stop
Feel better → Run again
Pain returns → Stop again
That’s not healing. That’s deconditioning.
Let’s break down when you can run with Achilles tendonitis, when you shouldn’t, and how to do it safely.
Achilles tendinopathy is not usually a tear. It’s not a pulled muscle.
It’s a load tolerance problem.
Your tendon became irritated because the load placed on it exceeded its current capacity.
And here’s the key principle most runners miss:
Tendons adapt to load.
When you completely rest the tendon:
– Mechanical stimulus decreases
– Collagen organization declines
– Tendon stiffness drops
– Calf strength weakens
– Load tolerance shrinks
So when you return to running weeks later, the same load feels even harder.
Rest may calm symptoms temporarily.
But it does not rebuild capacity.
The answer is: sometimes — if it’s modified correctly.
Instead of asking, “Should I stop running?” the better question is:
“How can I adjust my running so my tendon can adapt?”
I use a simple 3-part safety framework with runners.
Mild discomfort is acceptable.
Sharp, escalating pain is not.
Some stiffness at the start of a run that improves as you warm up is common.
What you don’t want is worsening pain throughout the run.
If you are changing your gait, protecting the leg, or visibly limping — stop.
Altered mechanics increase overload elsewhere.
Morning stiffness is common with Achilles tendinopathy.
But it should not be significantly worse than the day before.
If you meet these three criteria, you likely don’t need to stop running.
You need to modify running.
That’s very different.

Not all Achilles pain is the same.
Occurs about 2–6 cm above the heel bone.
Typically responds well to:
– Slow eccentric heel drops
– Heavy slow resistance training
Occurs where the tendon attaches to the heel bone.
This type involves compression against the heel.
That means:
– Deep stretching can irritate it
– Heel drops off a step may aggravate it
– Uphill running increases compression
For insertional pain:
– Perform heel raises to floor level only
– Avoid dropping below neutral
– Be cautious with aggressive stretching
Diagnosis matters because treatment is not one-size-fits-all.
If you’re in the safe zone, here’s how to modify your running.
Speed increases tendon load.
Keep runs conversational pace.
Inclines increase dorsiflexion and compression at the Achilles.
Flat routes are your friend during recovery.
Instead of running fewer days per week, consider:
– Keeping frequency consistent
– Reducing mileage per run
Example:
Instead of 5 runs at 6 miles, run 5 runs at 3 miles with a long run of 5 or 6 miles.
This maintains load exposure without overwhelming the tendon.
A typical starting point:
1 minute run
1 minute walk
Progress weekly:
2 minutes run / 1 minute walk
Then 2 minutes run / 30 seconds walk
This keeps load in the system while allowing recovery windows.

Running modification alone is not enough.
You must rebuild tendon capacity.
For midportion Achilles:
– Slow 5–6 second eccentric heel drops
– Heavy slow resistance (3 seconds up, 3 seconds down)
For insertional Achilles:
– Heel raises to floor level only
– Avoid dropping below step height
– Avoid aggressive stretching
Later stage:
– Add progressive resistance
– Introduce plyometrics
– Return to performance gradually
Tendons adapt to progressive loading, not avoidance.
There are times you need to pause running.
Stop if:
– Pain is above 5 out of 10
– You are limping
– Pain worsens week to week
– You cannot walk downstairs comfortably
That’s overload.
But most runners I see are not there.
They are in the yellow zone, which means modify, not quit.
I’ve seen this in hundreds of runners.
When they continue smart loading and progressive strength work, they not only resolve Achilles pain but they come back stronger.
The key is structure.
Not guessing.
Not emotional reactions.
Not total shutdown.
Structured progression.

Dr. Duane Scotti, DPT, PhD, OCS is a running physical therapist, run coach, host of the Healthy Runner podcast, and founder of Spark Healthy Runner and has been a leader in the rehab and running community for 20+ years.
Duane initially started running as an adult, wanting to add some cardio to his gym sessions. After having hip surgery, his doctor advised him to get off the treadmill and run outside. Without having fully recovered, Duane suffered his first running-related hamstring tendon injury and many others as a novice runner early on.

After 19 years teaching in higher education, Duane decided to step away as a Clinical Associate Professor of Physical Therapy to pursue his passion educating the running community. You can teach an old dog new tricks!!! I have never considered myself a runner and I knew I needed to be smart about it because I am no spring chicken. Spark preached getting strong and healthy to be injury free and they practice what they preach. I am soooo glad I made the investment in me! I finished my half marathon last weekend and couldn’t be any happier! I just ran my second half marathon, both with Duane’s coaching, and PR’d by over 9 minutes! I got all choked up in the last mile when I realized that I felt strong and could still kick it up a notch.
